Well-Child Visit

Your child needs well-child visits starting soon after birth. We offer these first few visits as part of our newborn care services to discuss growth, nutrition, safety, development and anticipatory guidance.

As your child gets older, they need a well-child visit at 1, 2, 4, 6, 9, 12, 15, 18, 24, 30, and 36 months. Starting when your child reaches the age of 3 years, they need a well-child visit once a year unless they have underlying physical or mental health issues that require more frequent checkups. You can also schedule a sick visit between wellness checks if your child becomes ill or injured.

For your convenience, please print and complete the necessary developmental or mental health screening forms below based on the coordinating age of your child at the upcoming visit.

Newborn, 2 month, 4 month, 6 month well-visit: Postnatal Depression Scale

9 month well-visit: Postnatal Depression Scale and 9 month ASQ

12 month well-visit (one year old): 12 month ASQ

18 month well-visit (1.5 year old): 18 month ASQ and MCHAT (Modified Checklist for Autism in Toddlers)

24 month well-visit (two year old): 24 mos ASQ

30 month well-visit (2.5 years old): 30 mos ASQ and MCHAT

11-17 year old well-visit: PHQ9-Modified for Teens

18-21 year old well-visit: PHQ-9
